Event Waiver

As a parent or guardian, I give permission for my child to participate fully in all activities associated with the All Saints' Anglican Church Nowra “Science Day” on 17th July 2026.

In the event of a medical emergency, I authorise the leaders of All Saints' Anglican Church Nowra to contact emergency services and arrange medical attention for my child. From that point onward, emergency services will liaise with me and with church representatives. Relevant medical information may be shared with third parties involved in my child’s care as needed.

I understand that every effort will be made to contact me before any medical procedures are carried out, in order to obtain consent. I acknowledge that All Saints' Anglican Church Nowra is not authorised to approve any invasive medical procedures. In the case of a minor medical incident that does not require emergency services, standard first aid may be administered. I will be contacted, or one of the alternate contacts listed, to advise of the steps taken.
